Treating Vitiligo with Tacrolimus Ointment in child

Vitiligo is a relatively very common dermatologic disease and one can easily observe it around his surrounding. Childhood vitiligo is a quite distinct subset of vitiligo, with high incidence of segmental vitiligo; family history of autoimmune or endocrine disease, early or premature graying of hair etc.

It occurred in all ages and can be psychologically devastating especially in children. Normally, children after diagnosed by vitiligo experience a range of feelings and emotions, including; shock, denial, anger, fear, guilt, anxiety, isolation, depression etc. Some children suffered with vitiligo, never really fulfill their potential in right way and came underachievement at school and other places.

Although, dermatologists are trying their best for treating this vexing disease, but they still have a long way to go in order to progress in this field. Normally, the mainstays of current therapy are topical steroids and phototherapy, either in combination or alone. But it is a limited treatment due to its potential for adverse effects.

Recently, investigators have reviewed the tacrolimus treatment of 57 pediatric people with vitiligo. They observed that, among 57 vitiligo people, 43 patients who undergone other treatment like, topical steroids and phototherapy (various PUVA or sunlight regimens) had produced little improvement. While other patients who received the tacrolimus ointment for at least 3 months showed repigmentation.

Thus from this observation physicians have evaluated that, tacrolimus had showed very best response in people with facial segmental vitiligo and had produced at least partial response in 84% body areas. It was also proved that tacrolimus ointment is one of the most valuable treatments for childhood vitiligo, particularly on the head and neck.

Treating Childhood Vitiligo with Topical Calcipotriol cream

After having inspired by reports of 1, 25-dihydroxyvitamin D3 receptors on melanocytes and its potential role in the regulation of melanogenesis; the clinicians of India conducted an open-label uncontrolled trial, to examine the efficacy of 0.005% topical calcipotriol cream and sun exposure. This experimental study was performed on 21 children with limited vitiligo having aged between 5 to 17 years old.

These children were advised to apply topical calcipotriol cream on vitiligo affected area, every evening. Similarly, they have to expose their body lesions to sunlight for 10 minutes, every morning. The efficacy of this treatment was regularly assessed with serial photographs, that were taken after every 6-week.

At the end of this activity, 18 children were available for final evaluation. Result showed that 10 children had shown 75% to 100% repigmentation, 4 had shown less than 25% improvement, while other had shown minimal or no improvement. The evidence of repigmentation was usually apparent within 3 months.

Thus, it was concluded that, the use of topical calcipotriol for treating vitiligo is intriguing and is less convincing. It was also informed that the bilaterally symmetric distribution of vitiligo will be well-suited to a self-control topical therapeutic trial in the future. Although topical calcipotriol was approved by the FDA in 1994 and has enjoyed several years of safe use in children with vitiligo and psoriasis, but it must be applied, sparingly and caution must be there in order to avoid its side effects.
